Our Enhanced Care Management (ECM) program offers comprehensive support through one dedicated person who helps you navigate health, social needs, and community resources, with personalized support and advocacy. ECM is free for people enrolled in Medi-Cal.
Care management means you are not navigating the system alone. You have another person committed to your well-being. We offer care that looks at the whole you: your health, your needs, and what's happening in your life.
Who can get ECM?
You may be eligible if you:

Have Partnership HealthPlan (Medi-Cal)

Have ongoing physical health, mental health, or life challenges

ECM provides direct support with:

Health & wellness. Setting and reaching your goals.

Navigating resources. Addressing barriers to your wellness.

Connection to services. Housing, food support, mental health care, and more.

General advocacy. When working with schools, social agencies, and healthcare systems.

Organizing care. Referrals, appointments, paperwork, and benefits.
ECM is offered alongside our therapy services. Rhizome provides care that is trans and queer affirming, neurodivergent affirming, non-pathologizing, trauma-responsive, and community-centered. We understand that health is connected to safety, housing, food, culture, and support, not just appointments and medication.
